During the Procedure Itself
Before any tooth removal begins, your surgeon will thoroughly numb the area with local anesthesia so you don’t feel pain during the extraction.
What you may notice is pressure or a sense of movement as the tooth is loosened and removed. Pressure is not pain, and it doesn’t mean the anesthesia isn’t working. It just means the tooth is coming out.
If at any point you feel something beyond that expected pressure, let your surgeon know. Comfort adjustments can be made quickly, and you won’t be made to feel like you’re interrupting anything. Your comfort during the procedure is part of the job.
Managing Discomfort as It Heals
Once the anesthesia wears off, some soreness around the extraction site is a normal part of healing. For most people, it peaks in the first day or two and then steadily improves. That arc matters to know, because feeling sore on day one doesn’t mean something is wrong.
Over-the-counter pain medication is typically enough to manage that soreness. Alternating with an ice pack during the first 24 hours can also help reduce swelling.

If soreness seems to be getting worse after a few days rather than better, or if you notice signs like fever or an unusual taste at the extraction site, those can indicate a dry socket or infection worth addressing. Neither is something to manage on your own when the practice is just a phone call away.

Molar extractions follow the same core approach as other extractions, but the position of these teeth and the structure of their roots means there’s more to account for compared to front teeth.
Upper molars tend to have roots that spread in multiple directions. Lower molars sit close to the jaw nerve, so your surgeon takes additional care to work around that anatomy. When a wisdom tooth is involved, especially one that hasn’t fully come through, the complexity increases. An impacted tooth doesn’t necessarily change what you feel during the procedure, but it can mean a few extra days of soreness before the site settles.
